New patients can fill out the initial paperwork ahead of time to speed up the first visit. Please fill out the Patient Registration Form, Treatment Consent, Financial Policy, Health Questionnaire, and remember to bring your insurance card. If you would like to obtain records from your previous treating physicians and providers, please fill out the Medical Information Release. The forms can be brought to your appointment or you can fax them to (805) 531-9499 ahead of time so we can enter the information into the system. Please do not email these completed forms.
